Dear Peninsula School District High School Families,
This is a reminder about the second dose vaccine for high school students who participated in the clinics on June 1.
Peninsula School District is hosting the Tacoma-Pierce County Health Department (TPCHD) to offer free Pfizer COVID-19 vaccines to students/staff ages 12 and up on-site at Gig Harbor High School (Auxiliary Gym) and Peninsula High School (Robotics Lab near Roy Anderson Field).
Second Dose Vaccine - Tuesday, June 22, 11:00 am - 3:00 pm
Appointments for this clinic are not necessary. Please have your child bring a new signed consent form and their shot record if this is their second vaccination.
This clinic is also open to any walk-in students or staff who would like a first or second dose, even if a first vaccine was given at another location.
Families of Track B students can choose for their student to be vaccinated during their in-person building day. All other families (Track A, Track C, or Track B students wishing to receive the vaccine after school) would need to provide transportation to/from school at a time that's convenient. District transportation is not provided for those who have a vaccination appointment time that is after school or while they are learning remotely.
In accordance with our safety protocols, visitors are not allowed in our school buildings. Families transporting their students are asked to wait in the parking lot while their student receives the vaccine. Healthcare providers from TPCHD will administer the vaccines and a licensed health care provider will be on site in the event of any reactions to the vaccine.
Families will need to review the Emergency Use Authorization form and complete the pediatric consent forms below. Adults and students over 18 can use this adult consent form.
