Dear Peninsula School District Middle School Families,

This is a reminder about the second dose vaccine for middle school students who participated in the clinics between June 14-28, 2021. Students already have their appointments and consent forms on file. 

If a student or staff member wants a vaccine and did not get one on the first round of shot clinics, they can participate in the clinic as a walk-in if they have a signed consent (attached below). If it is their second vaccine, they would need to provide their vaccine record of the first dose given.

Peninsula School District is hosting a third party pharmaceutical provider, Purdy Costless Pharmacy, to offer these free vaccines to children ages 12 and up on-site at our middle schools. The phone number for Costless Pharmacy is 253-857-7677 and families may contact them with any questions.

As mentioned in our last communication about middle school student vaccines, visitors are not allowed in our school buildings. Families of remote students are asked to wait in the parking lot while their student receives the vaccine. An emergency responder will be on-site in case of any reactions to the vaccine. 

District transportation is not provided for those who choose a vaccination appointment time that is after school.
